During the pre-commissioning checks of a kiosk substation, an ASP1 employee unbolted and removed the high voltage (HV) termination boots to conduct phase identification tests on the cables being connected to the network. Sometime following, the Access Permit recipient for the outage submitted a signed off Access Permit indicating “all works complete” to the Ausgrid Compliance Officer whilst onsite. Shortly after, the Compliance Officer identified the Equipping Permit had not been cancelled and went to the kiosk to obtain the Equipping Permit. It was at this time the Compliance Officer completed a final check of the substation, identifying the HV termination boots had not been installed correctly, leaving the HV terminations in the substation exposed.
